Cassandra’s Restoration with a High Smile Line and Poor Bone Quality
Background
Cassandra came to us with severe periodontal disease and a high smile line, meaning much of her gumline and dental work would be visible when she smiled — making aesthetics and precision especially important. On top of that, her bone quality was extremely poor, adding complexity to her treatment plan.
Challenge
For patients like Cassandra, poor bone density poses a major challenge for implant-supported restorations. With less bone to anchor into, implants may not achieve the stability required for long-term success. At the same time, Cassandra needed a solution that was secure, functional, and esthetically pleasing — and she wanted to leave the same day with teeth in place.
Solution
To meet Cassandra’s needs, we took a strategic and proactive approach. Using advanced digital planning, we prepared for an All-on-4 restoration — but due to her poor bone quality, we placed extra implants (All-on-7 or All-on-8 configuration). These additional implants served as backups, ensuring that if any one implant didn’t meet the stability threshold, another was available to support the restoration.
We explained to Cassandra that more implants doesn’t always mean better, but in unique cases like hers, it’s about customizing the plan for success — and having a safety net ready.
Surgical Experience
During surgery, we used a specialized computer system to test implant stability — similar to a tuning fork, it sends a vibration through the implant to measure how well it’s integrated into the bone. If the vibration is absorbed quickly (no sound or movement), we know the implant is strongly seated.
This process allowed us to quantify implant strength in real time, ensuring only the most stable implants were used to support Cassandra’s same-day bridge.
Outcome
Cassandra received a beautiful, immediate fixed bridge that was tailored not only to her smile line and esthetic needs, but to the structural limitations of her bone. Thanks to thoughtful planning, digital diagnostics, and surgical flexibility, she was able to leave with secure, natural-looking teeth — despite her complex case.
Key Takeaways
- Not all cases are All-on-4. Cassandra’s case highlights the importance of individualized treatment, where additional implants may be placed as backups — even if not all are used.
- Technology makes safer, smarter choices possible. Real-time implant stability testing helps us make informed decisions during surgery.
- Poor bone quality doesn’t mean you’re out of options. With the right planning and care, patients with compromised bone can still achieve full-arch implant success.
